The Parish Nurse Program focuses on assisting seniors to maintain their independence and optimal physical, mental and emotional health. The program provides services to seniors, lower income individuals and families, immigrants, and homeless clients at Saint Gabriel, San Isidro, St. Henry, Blessed Sacrament, Our Lady of Mercy Parishes and St. Laurence Chapel. Parish Nurse services include health individual and group education, health lectures, support and counseling, care navigation, and a focus on chronic disease management.
